The verdict

Otto, founded in 2015 as TeleVet and rebranded in 2023, bundles veterinary client communication into one price: unlimited two-way texting, reminders, follow-ups, lapsed client campaigns, refill requests, forms, check-in, direct booking, telemedicine, team chat and a conversation board, plus the AI Suite for SOAP notes, call recaps and suggested replies. Otto Pay adds text and chat to pay, deposits and PIMS-connected terminals, Otto Care adds membership plans, and Otto Phones adds integrated calling. It syncs with Cornerstone, Neo, AVImark, Pulse, ezyVet and ImproMed in under five minutes. Pricing runs from $350 to more than $1,000 a month locked for a year, with a standalone AI Scribe from $49. The catches are the price, the annual commitment and thin independent review coverage.

How Otto works day to day

Otto is an all-in-one veterinary client communication and workflow platform that puts reminders, two-way texting, digital forms, online booking, payments, memberships, phones and AI scribing behind one subscription. It started in 2015 as TeleVet, a veterinary telemedicine company founded in Austin, Texas, by Steven Carter and Price Fallin, and rebranded as Otto in August 2023 alongside a $43 million Series B, reflecting its move beyond telemedicine. The company says more than 10,000 veterinary professionals use it, and its in-house veterinarians include Chief Veterinary Officer Dr. Jennifer Quammen.

Otto Flow is the core product. One monthly price includes unlimited two-way texting, email campaigns and templates, automatic service and appointment reminders, automated follow-up messaging, lapsed client automations, prescription refill requests, client surveys and reviews, customizable digital forms, a website widget, automated check-in, direct booking where your PIMS supports it, telemedicine appointments, photos and video in chat with multiple pet parents in one conversation, internal team chat and task management. A conversation board shows open messages, checked-in patients and appointment requests side by side, so the front desk can see the whole day at a glance. Otto says clinics reduce incoming calls by about a third; one manager tracked it and reported exactly that.

The AI layer is included with Flow. The Otto AI Suite records appointments and writes SOAP notes, transcribes and recaps phone calls for follow-up and training, and drafts suggested replies to client messages, with optional writeback to the PIMS. OttoPilot, the AI engine behind the platform, adds business insights and recommendations, and agentic confirmations automate appointment follow-up. For practices that only want notes, the standalone Otto AI Scribe starts at $49 a month with the first 21 days free, uses a vet-trained model that understands abbreviations, medications and shorthand, supports SOAP, narrative and bullet templates and runs on Apple and Android, with notes ready about a minute after the appointment.

Payments and memberships are where Otto goes further than most communication tools. Otto Pay covers text to pay, chat to pay, saved cards, digital payments, pre-appointment deposits and terminals connected to your PIMS, with payments written back to the record. Otto Care lets you build customized membership and wellness plans with flexible credits, and Otto cites up to 55 percent more visits from plan members. Otto Phones adds an integrated phone system, so calls, texts and payments sit in one place.

Integration is deep with the major systems. Otto lists Cornerstone, IDEXX Neo, AVImark, Covetrus Pulse and eVetPractice, ezyVet and ImproMed, and claims sync times of under five minutes, so appointments, patients, notes, forms and payments stay current. Onboarding is run by implementation specialists who have worked in practice, and each clinic gets a dedicated customer success manager after launch.

Otto pricing is published as a range rather than a rate card: one communications platform price runs from $350 to more than $1,000 a month, locked in for a year, with special pricing and discounts for many clinics. That price includes the full Flow feature set and the AI Suite with no upgrade-to-unlock tiers or overage fees, while Otto Pay processing, Otto Care and Otto Phones are separate products. The standalone AI Scribe Solo plan starts at $49 a month.

The trade-offs are cost and commitment. At $350 a month and up, Otto is more expensive than modular tools such as VitusVet, the annual price lock means a year-long commitment, and direct booking depends on what your PIMS supports. Independent review coverage is thin on Capterra, so ask for references; VetSoftwareHub lists Otto at 4.8 out of 5. Competitors, including IDEXX, also argue that some workflows still require toggling between Otto and the PIMS, so test your busiest tasks in the demo.

If Otto is not the right fit, the closest Otto alternatives are PetDesk for vet-only communication with the widest PIMS support, VitusVet for modular reminders and payments, IDEXX Vello for ezyVet, Neo and Cornerstone practices, Weave for phones, texting and reviews, and Mango Voice if you mainly need a phone system with an AI receptionist. The verdict: Otto is the most complete single subscription for veterinary client communication, payments, memberships and AI, and the best fit for busy practices that want to replace several tools with one platform and one price.

How to test Otto before you commit

Every tool demos well. These are the checks that actually tell you whether Otto fits your practice, while the free trial is still running.

Run a real appointment end to end

  • Use your own templates and one of your busiest appointment or session types
  • Test it on the devices your staff actually use, not a demo laptop
  • Move the result into the system you already use and time every step
  • Add a second user and watch how the per-seat price moves

Watch for the catch

  • Usage caps, credits or add-on fees that only show up on the invoice
  • Features quietly locked to a higher tier than the one you tried
  • Onboarding or data import sold as a paid add-on
  • How hard it is to export your data if you decide to leave
